    Eligibility is based on:

    Mission: To assist people to remain in their homes.

      Poor No charge < $1,500 assets
      Mid Small charge <$17,400/person
      or $26,100/couple
      Affluent Fee for service >$26,100/couple

      Rarely turned down for this program.

    2. Functional Skills (ADLs)

      Bathing
      Meals
      Shopping
      Medication compliance
      Dressing

    3. Services That Can Be Paid For:

      Home Health Aide
      Adult Day Care
      Homemaker
      Meals on Wheels
      Emergency Response
      Companion
      Chore Service

  2. Alzheimer's Respite Program

    Provides Dollars/year to be used for:

    1. Hours/day
    2. 1-4 days at a time

    Mission: Respite for caregiver to take care of their personal, recreational, or emotional needs.

    No financial threshold for admission to program.
