Your Appointment
If this is your first visit with us, or if you are returning to our care, please complete our registration and consult forms.
Please remember to bring the following with you to your appointment:
- Insurance card
- A list of current prescription and/or over-the-counter medications, including the dose and frequency
- Pertinent information about your neurological history
- Any recent X-rays, MRIs, CAT scans, bloodwork, or other test results sent by your primary or referring health care provider
- A list of questions you would like to ask the doctor
Payment and Insurance Information
- Medicare
- Most HMO and PPO plans
We are providers for most major insurance plans. However, it is in your best interest to check your insurance provider's list of participating physicians to be sure that your plan allows you to be seen at our office. Also, please review the rules and limitations on coverage and reimbursement with your insurance provider.
We are happy to process all insurance forms for you as long as you have provided us with the necessary insurance information. We expect prompt payment if you receive a bill from our office for balances that may be due. Please feel free to discuss any questions you may have about billing with office staff.

Hospital Affiliations
We are affiliated with Danbury Hospital in Danbury, Connecticut. Danbury Hospital is a 371-bed not-for-profit teaching hospital that serves approximately 360,000 residents of western Connecticut and southeastern New York. The hospital's specialized facilities include a Level II trauma center and the J. Benton Egee, MD, Emergency Department, The Praxair Cancer Center, a Level II neonatal intensive care unit, cardiac catheterization lab, the Marcus Cardiac Rehabilitation Center, the Duracell Center for Ambulatory Surgery, magnetic resonance imaging, CT scanning, 2 linear accelerators, Main Street Physical Rehabilitation Center, the Seifert & Ford Family Community Health Center, and a sleep disorders center.
Danbury Hospital has 10 clinical departments, 9 of which are directed by full-time chairmen. Additionally, there are 115 full-time physicians employed through the Danbury Office of Physician Services, PC (DOPS), all of whom meet medical staff bylaw requirements.
The hospital participates in residency training programs, which are actively supported by many community-based physicians, including dental, obstetrics/gynecology, medicine, nuclear medicine, psychiatry, pathology, pediatrics, and surgery. Danbury Hospital is the Western Connecticut Affiliate of the Yale Cancer Center and enjoys clinical affiliations with Yale University School of Medicine, the University of Connecticut School of Medicine, and New York Medical College.
